RESUMO

Objective: Compare the breast cancer mortality rate and the rate of mammograms with socioeconomic factors, in Brazilian´s Federative Units, during the period of 2015 to 2021. Methods: This is an ecological study, of a descriptive nature, which covers the 27 Brazilian federative units, using secondary data extracted from the DATASUS System. The information analyzed was mammography data, breast cancer mortality, demographic and economic data of the resident population and women over 20 years of age. Results: There is a direct relationship between mortality rate and mammograms rates in the Federative Units. The data obtained for Brazil presents a total of 133,048 deaths from breast cancer and 17,324,526 mammography exams in the period from 2015 to 2021. In other words, Brazil presents a standardized mortality rate value, 19.25 deaths per 100,000 women. Regarding the mammography rate, the value of the exam-specific mammography rate was 2,506.55 per 100,000 women. In relation to socioeconomic data by mortality rate group, higher mortality rates from breast cancer present higher rates in the variables: income rate by State (2,594.86), people with income (63.5), average percentage of women with more over 50 years old (29.5%), women with average years of education (6.64), white population (55.1%) and mammography exam rate (2873.20). When analysing the highest rates of mammography exam rates, higher rates are observed income rate by State (2,161.47), people with income (62.5%), average percentage of women over 50 years old (28.6%), women with an average number of years of education (5.8). Conclusion: Brazil has a high mortality rate due to breast cancer, when compared to other countries. The reasons are diverse, such as the modern, industrialized and populous lifestyle. The Federative Units with the highest mortality rate also have high rates of mammograms, that is, in the Federative Units with "better" sociodemographic conditions, the South and Southeast.

PURPOSE: The aim is to verify the impact of the Covid-19 pandemic on women's healthcare and medical assistance in Brazil. PATIENTS AND METHODS: This exploratory cross-sectional study evaluated a non-probabilistic sample of women above 20 years old, carried out between August and September of 2020, through a snowball sampling using a Google Forms application. RESULTS: From a total of 2495 women, more than 70% have not been screened for cervical cancer (77.8% of 2244 women aged for screening), and more than 80% have not been screened for breast cancer (80.2% from 1325 women aged for mammography) during the pandemic. Also, 55.2% of the women did not undergo routine blood tests during the same period. The most frequent reasons for not performing screening and routine tests were: they were up to date; fear of contracting Covid-19; they decided to postpone it until after the end of the pandemic; they were unable to schedule the appointment at the healthcare center for whatever reason; and the healthcare center was only attending Covid-19 patients. Women with no comorbidities have performed significantly more mammograms and routine blood tests than women with comorbidities. In addition, women with comorbidities who were used to perform periodic medical follow-up have done it substantially more than women with no comorbidities during the pandemic. CONCLUSION: As observed, there was a significant decrease in women's access to the healthcare system during this pandemic. Many participants reported that they had not attended any screening tests, and some reasons included fear of getting infected and due to the public measures of social distancing. The consequences are late diagnoses and a worse prognosis. It might impact the healthcare systems around the world in the next few years. Further studies should be done to follow these consequences.

BACKGROUND: The increase in violence against health professionals in the COVID-19 pandemic makes it necessary to identify the predictors of violence, in order to prevent these events from happening. OBJECTIVE: Evaluating the prevalence and analyzing the variables involved in the occurrence of violence against health professionals during the COVID-19 pandemic in Brazil. METHOD: This is a cross-sectional study conducted online involving Brazilian health professionals during the COVID-19 pandemic. The data were collected through a structured questionnaire (Google Online Form) sent to health professionals on social networks and analyzed through logistic regression by using sociodemographic variables. The set of grouped variables was assigned to the final model when p <0.05. A network was built using the Mixed Graph Models (MGM) approach. A centrality measurement chart was constructed to determine which nodes have the greatest influence, strength and connectivity between the nodes around them. RESULTS: The predictors of violence in the adjusted regression model were the following: being a nursing technician / assistant; having been working for less than 20 years; working for over 37 hours a week; having suffered violence before the pandemic; having been contaminated with COVID-19; working in direct contact with patients infected by the virus; and having family members who have suffered violence. The network created with professionals who suffered violence demonstrated that the aggressions occurred mainly in the workplace, with an indication of psycho-verbal violence. In cases in which the aggressors were close people, aggressions were non-verbal and happened both in public and private places. The assaults practiced by strangers occurred in public places. CONCLUSIONS: Violence against health professionals occurs implicitly and explicitly, with consequences that can affect both their psychosocial well-being and the assistance given to their patients and families.

O tumor sólido-cístico pseudopapilar do pâncreas (TSCPP) - tumor de Frantz - é uma neoplasia rara do pâncreas que ocorre principalmente em pacientes jovens e apresenta um bom prognóstico. Pouco mais de 700 casos foram relatados na literatura mundial. A maioria das publicações estudadas relatam casos deste tumor em pacientes do sexo feminino, que ao diagnóstico apresenta-se oligossintomáticas ou assintomáticas, com massa abdominal palpável, em que a tomografia computadorizada evidencia presença de massa sólidocística. Dois terços destes tumores ocorrem no corpo e cauda do pâncreas e, apesar do crescimento excessivo para fora dos limites pancreáticos, raramente invadem estruturas vasculares ou órgãos adjacentes. A maioria dos pacientes são curados com a ressecção completa do tumor, porém 10 a 15% apresentam metástase sincrônica ou metacrônica. Em nossa casuística, um deles apresentou-se de forma clássica: mulher jovem, oligossintomática, submetida à pâncreatectomia corpo-caudal com preservação do baço, que obteve cura com a ressecção da lesão. No segundo caso, paciente do sexo masculino, sexagenário, submetido a ressecção corpo-caudal do pâncreas, apresentou recidiva hepática dois anos após tratamento da lesão primária e foi submetido à ressecção da lesão metastática do fígado. Ambos atualmente em acompanhamento e sem evidência de doença. Enfatiza-se a necessidade de se considerar o tumor de Frantz no diagnóstico diferencial de massa abdominal, sólido-cística em topografia pancreática e com crescimento indolente e oligossintomático. O objetivo deste trabalho é mostrar dois casos tumor sólido-cístico psudopapilar do pâncreas, sendo um caso típico e outro atípico: homem, sexagenário que evoluiu com lesão metastática


The pseudopapillary solid-cystic tumor of pancreas (TCPP) - Frantz tumor - is a rare neoplasm of the pancreas that occurs mainly in young patients and has a good prognosis. Just over 700 cases have been reported in the world literature. The majority of studied publications reported cases of female patients, introducing itself oligosymptomatic or asymptomatic with palpable abdominal mass, where the CT scan shows the presence of solid-cystic mass. Two-thirds of these tumors occur in the body and tail of the pancreas, and despite the excessive growth beyond pancreatic limits, it rarely invaded adjacent organs or vascular structures. Most of patients are cured with complete tumor resection, but 10 to 15% have synchronous or metachronous metastasis. In our series, one of them have presented a classical form: young woman, oligosymptomatic, submitted to pancreatectomy body-caudal with preservation of the spleen, which achieved cure with resection of the lesion. In the second case, the male patient in his sixties, subjected to body-tail pancreatic resection, liver showed recurrence two years after treatment of the primary lesion and underwent resection of metastatic liver damage. Currently both are in monitoring and have no evidence of disease. It emphasizes the need to consider the Frantz tumor in the differential diagnosis of abdominal mass, especially when in pancreatic topography and indolent growth and oligosymptomatic. O carcinoma inflamatório de mama, tradicionalmente, tem sido associado a um mau prognóstico devido a seu crescimento rápido, alta freqüência de metástases a distância e progressäo locorregional. O comportamento biológico do carcinoma inflamatório é sempre um desafio para os oncologistas, sendo seu tratamento multimodal. O objetivo do estudo foi a análise retrospectiva de 34 casos e a correlaçäo de variáveis clínicas e epidemiológicas com o prognóstico. Foram avaliadas 34 pacientes no período de janeiro de 1990 a dezembro de 1997, correspondendo a 1,7 porcento dos casos de câncer de mama. Os autores observaram a alta freqüência de metástases a distância e a correlaçäo do comprometimento linfonodal com essas metástases e uma sobrevida de 34,5 porcento e 20,7 porcento, em 3 e 5 anos, respectivamente
